That should help. Frequently it makes things harder.
When you already understand it, you lose the comfortable explanation. You cannot tell yourself you just need more insight, because you have plenty. So the only remaining conclusion appears to be that something is wrong with you specifically, or that you are not trying hard enough, which for someone who tries hard at everything is a demoralizing place to land.
Here is what is actually happening. Insight and change run on different systems. Understanding lives in the part of you that reasons. The reaction lives in the part that fires before reasoning arrives. You can hold complete clarity about a pattern while your nervous system runs the old response, because your nervous system does not process arguments. It updates through experience.
That is not a character problem. It means the work has to happen somewhere other than the level of explanation, and most therapy stops at the explanation.
What the Work Actually Is
I will be specific about this, because vagueness on this point is what makes people put off calling.
Learning to catch it early enough to matter. Not recognizing the spiral afterward, which you can already do. Catching it in the few seconds before the response fires, which is a trainable skill and does not develop without practice.
Having genuine choice about the thought. There are two separate skills here. One is going into it deliberately: taking hold of the thought, examining what it is actually claiming, and shifting the angle you are viewing it from. The other is recognizing the entrance to a spiral and deciding not to go in. Some things are worth working through. Others are not worth the hour they will cost you, and knowing the difference is most of the value. What matters is that both are available, so the spiral becomes a decision rather than an event.
Building the alternative in advance. Seeing a pattern with nothing else to do just means watching yourself do it. So we work out the different response ahead of time and rehearse it, because the moment itself moves too fast to improvise in.
Changing the state underneath. A body that has been braced for years does not settle because you understood something. That is nervous system regulation work, and it is what makes a change hold rather than last three weeks.
Boundaries that survive contact with reality. Not scripts. The actual skill is tolerating someone being disappointed in you without immediately fixing it, which is a physical experience before it is a decision.

The specifics differ. The structure underneath is remarkably consistent.
Therapists and clinicians. You spend your days holding other people’s material and have nowhere to put your own. You know exactly what you would say to a client in your position, which somehow makes it harder rather than easier to bring anywhere.
